Wine Linked to Lower Heart Death Risk — Until Alcohol Intake Gets High, Study Finds Life

A large study of British adults has found a striking divide in the health outcomes associated with different types of alcoholic drinks. Heavy drinking was linked to a higher risk of death regardless of what people drank, but at lower levels of consumption, wine showed a markedly different pattern from beer, cider and spirits.

Researchers analyzed data from 340,924 participants in the UK Biobank, tracking their health for more than 13 years on average. The study examined both the quantity of alcohol consumed and the type of beverage, allowing researchers to compare mortality patterns among wine drinkers and people who primarily consumed beer, cider or spirits.

The clearest finding concerned heavy alcohol use. Compared with people who drank rarely or not at all, participants classified as heavy drinkers had a 24% higher risk of death from any cause, a 36% higher risk of dying from cancer and a 14% higher risk of cardiovascular death. Those increases were seen regardless of the type of alcohol consumed.

At low and moderate consumption levels, however, beverage choice was associated with substantially different outcomes.

Wine showed a different mortality pattern

Moderate wine consumption was associated with a 21% lower risk of death from cardiovascular disease compared with never or occasional drinking.

Beer, cider and spirits moved in the opposite direction. Even at relatively low consumption levels, these beverages were associated with a 9% higher risk of cardiovascular death compared with people who rarely or never drank. Researchers also found that low or moderate consumption of beer, cider and spirits was associated with higher overall mortality, while comparable levels of wine consumption were associated with lower mortality.

The findings do not establish that wine itself prevents heart disease.

The study was observational, meaning researchers identified associations rather than proving that a particular beverage caused better or worse health outcomes. Drinking habits were also reported by participants themselves when they entered the UK Biobank, and changes in alcohol consumption during the years that followed were not captured.

What counted as low, moderate and heavy drinking?

Researchers measured consumption using grams of pure alcohol rather than simply counting glasses or bottles.

For comparison, approximately 14 grams of pure alcohol is found in a 12-ounce beer, a 5-ounce glass of wine or a 1.5-ounce serving of spirits.

Participants consuming less than 20 grams of alcohol per week were classified as never or occasional drinkers.

For men, low consumption ranged from 20 grams per week to 20 grams per day, while moderate consumption was between 20 and 40 grams per day. More than 40 grams per day was classified as high consumption.

For women, low consumption ranged from 20 grams per week to 10 grams per day, moderate consumption was between 10 and 20 grams per day, and more than 20 grams per day was considered high.

The wine effect may not be entirely about wine

One possible explanation involves compounds naturally present in wine, particularly red wine. Polyphenols and other antioxidant compounds have previously attracted attention for their possible cardiovascular effects.

But the researchers highlighted another explanation that may be just as important: wine drinkers may differ from beer and spirits drinkers in ways that extend well beyond what is in their glass.

Wine was more commonly consumed with meals and was associated with better overall diets and healthier lifestyle behaviors. Beer, cider and spirits were more frequently consumed outside meals and were associated with poorer diet quality and other lifestyle risk factors.

Researchers attempted to account statistically for a wide range of differences, including socioeconomic status, demographics, lifestyle, cardiometabolic health and family histories of diabetes, cardiovascular disease and cancer. Even extensive statistical adjustments, however, cannot completely eliminate the possibility that unmeasured lifestyle differences contributed to the results.

That distinction matters because an apparent benefit among wine drinkers does not necessarily mean that switching from beer to wine would produce the same benefit.

More than the amount in the glass

The research adds another layer to the long-running debate over whether low levels of alcohol consumption can ever be considered beneficial.

Zhangling Chen, a professor at the Second Xiangya Hospital of Central South University in China and the study’s senior author, said the results suggest alcohol-related risk may depend not only on the quantity consumed but also on the beverage and the behaviors surrounding its consumption.

The study also carries an important limitation specific to the UK Biobank. Participants in the database tend to be healthier than the British population as a whole, which means the findings may not translate perfectly to other populations.

What the research does establish more firmly is the danger at the upper end of consumption. Once drinking reached the study’s high-consumption category, the apparent differences between beverages became far less reassuring: heavy alcohol use was associated with increased mortality whether the drink was wine, beer or spirits.

The more provocative question lies below that threshold. Among hundreds of thousands of adults followed for more than a decade, the type of alcohol people chose was associated with substantially different long-term outcomes.

Whether those differences come from the drink itself, the circumstances in which it is consumed, the lifestyles of the people who choose it, or a combination of all three remains unresolved.
